Incident Reporting Guidelines

•  Serious illness, injury, or accident requiring emergency hospitalization
• Unplanned hospitalization or psychiatric admission
• Death of an individual
• Physical aggression causing injury
• Suicide attempts
• Medication errors resulting in harm or risk
• Missing person incidents per the ISP timeframe
• Fire requiring fire department response

 Abuse or neglect must be reported immediately to case management, DHS, or law enforcement. 

•  Immediate Notification: Within 1 business day to the case manager for serious events.
• Written Report: Due within 5 business days.
• Copies must be maintained in the client file and provided to legal/designated representatives upon request.
